Homeside Hospice, a nurse owned and operated hospice, is looking for a highly motivated, compassionate, and energetic candidate for Full Time RN, Case Manager to work out of Union, Middlesex, Somerset and Mercer counties. Must have a current and valid license to practice professional nursing in the state of NJ. Preference of at least two (2) years of work experience as a Hospice nurse. Demonstrates professional competency, ability to assess and respond to the needs of patients and families in varied settings, and good organizational skills. Responsible for the maintenance and restoration of health; prevents infection, accident and injury; performs an initial assessment and identifies problems for each patient upon admission to hospice; reassesses the patient’s nursing care needs on an ongoing, patient-specific basis and provides care which is consistent with the Plan of Care; and monitors the patient’s response to nursing care. Also responsible for teaching, supervising and counseling the patient, family members, and staff regarding nursing care and the patient’s needs, including other related problems of the patient at home. Must be willing to travel multiple counties in NJ, will be compensated for paid tolls and mileage. Director of Clinical Development Opportunity Description
The Home Care and Hospice Association of New Jersey is looking for a Director of Education to join their team. This position is preferably based in Cranford, NJ, however, a NJ based remote candidate may be considered. An RN with an education or training background in Medicare Home Health is a must. Knowledge of New Jersey Home Health and Medicaid rules and regulations is strongly preferred.
